Care Coordination provides advocacy and person-centered planning to connect individuals with preferred and needed medical, social and habilitation services.
Services:
Assist individuals and families in developing an appropriate plan of care
Connect individuals and families with local service providers
Monitor the quality and frequency of delivered services
Advocate for individual and family needs
Evaluate service outcomes
MSSCA Care Coordination services are provided to eligible individuals per the State of Alaska Administrative Code 7AAC 43.1041.
Eligibility:
Eligibility for services is determined by the Division of Senior and Disabilities Services (DSDS) according to AS 47.80.900(7) (A-E).

Medicaid Home and Community Based Waiver (HCBW)
Private Pay
Cost:
Costs of services paid for through Medicaid HCBW. Individuals must first be selected or “drawn” for a Medicaid HCBW by the State of Alaska, DSDS.
