Care Coordination

Care Coordination

Care Coordination provides advocacy and person-centered planning to connect individuals with preferred and needed medical, social and habilitation services. 

Services:

  • Assist individuals and families in developing an appropriate plan of care
  • Connect individuals and families with local service providers
  • Monitor the quality and frequency of delivered services
  • Advocate for individual and family needs
  • Evaluate service outcomes

MSSCA Care Coordination services are provided to eligible individuals per the State of Alaska Administrative Code 7AAC 43.1041.

Eligibility:

  • Eligibility for services is determined by the Division of Senior and Disabilities Services (DSDS) according to AS 47.80.900(7) (A-E).

Sources of Funding:

  • Medicaid Home and Community Based Waiver (HCBW)
  • Private Pay

Cost:

  • Costs of services paid for through Medicaid HCBW.  Individuals must first be selected or “drawn” for a Medicaid HCBW by the State of Alaska, DSDS.
